Emergency Temporary Permit Application

List of APPROVED Emergency Temporary Permits for Health Care Practitioners Evacuating from Florida

The LSBME’s rules, La. Admin C. 46.XLV.412, provides for the issuance of emergency temporary permits for physicians and allied health care practitioners, to provide voluntary, gratuitous medical services during a public health emergency. The permit may be issued provided the applicant (1) holds a current, unrestricted license in good standing issued by the licensing authority of another state to practice the profession for which the permit is sought; (2) presents indisputable personal identification; (3) presents a copy of the professional license or information which can be used to verify out-of-state licensure; (4) completes an application.

In response to the hurricane in Florida, the Board will issue 60-day emergency temporary permits to physicians and allied health care practitioners who will be temporarily be working in Louisiana as a result of the evacuation. The permit will expire 60 days from the date it was issued, unless the practitioner requests an extension, and the Board, in its discretion, may extend the permit for an additional 60-day period.

The one page application must be submitted via email to licensing@lsbme.la.gov or faxed to 504-599-0503.  The Board will use the information provided in the application to verify the practitioner’s credentials with the Florida Board. The completed application needs to be accompanied by a copy of the practitioners drivers license, or other means of indisputable personal identification.  Finally, the Board must be in receipt of written verification from the hospital where the physician will be working indicating he/she has been granted hospital privileges.

Once these documents are received, and the Board has verified that the applicant’s Florida credentials are in good standing, the Board will issue the temporary permit at no charge for an initial period of 60 days.

A list of providers who are issued temporary permits, along with the permit expiration dates, will be maintained above for public access.
